Billing and insurance payments can sometimes be confusing. Billing agents are available from 8:30 a.m. to 4:30 p.m. (mountain standard time) to help with questions, or to assist you in making payment arrangements.

We accept many other insurances: If you do not see your insurance carrier listed, simply ask us to check for you when you make an appointment. When you give us complete and accurate insurance information, we will submit claims to your carrier and bill you only for any uncovered charges.

However, you may also be personally responsible for co-payments, co-insurance, or other deductible amounts, and your insurance company may set limits on the doctors you can see, or the services you can receive.

Self Pay

For those without insurance, SFI offers half-price at the time of service or various payment plans. Please contact our office to discuss.
