Thank you for completing the appointment request form. Please allow for 24-48 hours for someone from SFI to call and schedule your next visit.

Here are some quick notes:

• Please ensure that you have orders from your provider. If you do not have orders, then we cannot schedule you.
• Please check with your insurance to see if your exam is covered.
• If you need an X-ray exam you can just walk-in to either of our Santa Fe locations. Here are our hours for X-rays for both locations:
  • Main:
    • Monday – Thursday: 8:00AM to 5:00PM
    • Saturday: 8:00AM to 12:00PM
  • Southside:
    • Monday – Sunday: 8:00AM to 6:00PM
  • Los Alamos @Trinity Urgent Care
    • Monday – Sunday 9:00AM to 6:30PM
• If you need to schedule a mammogram, please ask about same or next day appointments.
